Long Term Conditions Joint Clinic

  • Our practice nurses Kath Baddeley & Pat Ofori ad well experienced in looking after patients with long term medical conditions. Both nurses do annual review of long term conditions, with support of the duty GP (COPD, Asthma, Diabetes, CKD, Hypertension, and frail elderly checks)
  • As the duty GP is involved in this joint clinic, patients treatment is optimised at the same time without the need for the patient to come back for a second appointment.
